If you are planning to send in your kit please fill out and submit the following form.

This will pre-register your kit so we will know it is on the way. If you have sent your kit in before you can fill out the short form if your contact information has not changed. When we receive your kit we will recertify and or calibrate it unless it requires repairs. If repairs are required we will call you with an estimate of the charges and or get a warranty from Mid-West to cover the charges if the unit is under warranty.
